I will create an attractive HTML form for you

About:

I'll design responsive HTML CSS Form with all input types. The form structure will be

Page Title
Top Headings (Form Name/website Name) {Optional}
Form Fields (With placeholders or Labels)

I'll provide you with a zip file which contains index.html page, Images (If any), CSS and JS files.
I'll use method "GET" or "POST" which one you prefer. I'll use action file address that you'll mention. By default Action file will be "index_ac.php".

Note: I'll just design form and will not use any back end script.
